What is urinary incontinence?

Urinary incontinence is the involuntary leakage of urine that occurs when you lose control of your bladder and can be particularly problematic when running, jumping, sneezing, laughing or coughing.

Some incontinence is considered a normal part of ageing in which the muscles supporting your bladder tend to weaken and lead to urinary incontinence.

However, the severity ranges from occasionally leaking urine doing physical activity to having an urge to urinate that's so sudden and strong and sometimes enough to wet the floor.

What treatment do you now offer for female urinary incontinence?

What precautions should I take before my laser procedure for incontinence? 

Make sure you tell your doctor of any medications you’re on - prescribed, over the counter or herbal, before your procedure.

Stop smoking for at least 10 days before your procedure and during the healing phase as smoking can impact recovery and results.

Patients are generally advised not to take aspirin or blood-thinning medications at least 1 week before laser therapy.

How effective is laser therapy for incontinence?

A growing groundswell of medical studies shows that laser may be a viable solution for mild to moderate urinary female incontinence.

A  2020 study in the International Journal of Obstetrics and Gynaecology* which looked at 31 studies on 1530 women, found that “all studies on vaginal and or urethral laser application for pelvic organ prolapse and urinary incontinence reported improvement.”  

It also found that no major adverse events were reported, with mild pain and burning sensation the most commonly prescribed adverse events.   

Another smaller study reported in 2019 BioMed Research International* confirmed that “vaginal laser is an effective treatment for women with mild to moderate urinary incontinence with no severe or permanent side effects during treatment.” 

However as laser therapy is still considerably new, evidence is weaker than established surgical procedures, and a systemic review in the Obstetrics and Gynaecology International Journal*  found that more research is required to further apply these technologies in clinical daily routine. 

However, it did conclude that “laser therapy seems to be a promising alternative approach to urinary incontinence in women. It seems effective in the short term, well-tolerated, with few, rare and transient side effects.” 

For severe incontinence however, surgery remains the first choice.

What are the symptoms of incontinence?*

The main symptom of incontinence is a leakage of urine. Incontinence is generally divided into four types: stress, urge, overflow, and mixed type. Depending on the type of incontinence you have, you might experience leakage for a wide variety of reasons.

 

Stress incontinence

  • Stress incontinence is triggered by certain types of physical activity.
  • This muscle weakness means you are more likely to lose control of your bladder while you are exercising, coughing, jumping, laughing, or doing any activities that put stress on the pelvic floor muscles
  • In this type of incontinence, your pelvic floor muscles are weak and no longer support your pelvic organs as they should.

 

Urge incontinence

  • This type of incontinence is characterized by an intense need to urinate right away. 
  • The loss of urine with a strong desire to urinate happens too suddenly for you to make it to a toilet.

 

Overactive bladder causes a frequent and sudden urge to urinate that you may always feel a strong urge to urinate all day, even waking up at midnight to urinate.

  • The causes can include weak muscles, nerve damage, use of medications, alcohol or caffeine, infection, and being overweight

 

Overflow incontinence

  • With this type of incontinence, you may experience constant dribbling of urine over time instead of one big gush of urine.
  • This inability to control urination occurs when you are unable to empty your bladder. 
  • This type of incontinence is more common in people with chronic conditions like multiple sclerosis (MS), stroke, or diabetes.
  • This may also occur in men with a large prostate that stops the flow of urine out of the bladder.

 

Mixed incontinence

  • Mixed incontinence is a combination of types that you may experience more than one type of urinary incontinence. The combination of stress incontinence and urge incontinence is the most common one.
  • Identifying what triggers mixed incontinence is usually the best way to manage the condition.

What causes incontinence?

Many different health problems can also cause the condition. The causes and the type of incontinence are closely linked types or vary depending on gender. Those causes can range from mild to severe. Some of them are temporary that usually stop once the condition is treated. However, chronic conditions usually don’t go away after a long period of treatment.

 

Temporary causes of incontinence include:*

Pregnancy and childbirth, as pressure from the expanding uterus can overwhelm the bladder. A baby also puts extra pressure on the bladder as it grows. Sometimes pregnancy-related incontinence will resolve after birth, but often it does not.

Drinking too much fluid, especially certain beverages such as alcohol, tea, and coffee.

Urinary tract infections can irritate your bladder, causing you to have strong urges to urinate.

Incontinence can also be a side effect of certain medications, including antidepressants and diuretics.

Incontinence tends to become common around menopause, due to dropping oestrogen

Constipation can also affect bladder control and urinary continence.

Other chronic causes of incontinence include:

Stroke, diabetes, MS, menopause, an enlarged prostate and pelvic floor disorders that can impact the way the urinary system functions. 

What are the “lifestyle” treatments for incontinence?

Bladder training can strengthen pelvic floor muscle and delay urination after feeling the urge to go.

Double voiding is a way to help you learn to empty your bladder more completely to avoid overflow incontinence.

Scheduled toilet trips to empty your bladder on a regular schedule may help.

Avoiding drinking caffeine or a lot of fluids.

Eating a healthy diet and maintaining a healthy body weight.

Wearing pads and products designed to catch leaked urine.

What medications are used for incontinence? 

Several medications can be used to stop the bladder being overactive (anticholinergics). 

Topical oestrogen may be used to ease symptoms, and sometimes tricyclic antidepressants are used for mixed incontinence to help relax the bladder neck.
